It is very important that you verify with your health plan that the physician you choose to see at Family Practice Associates is a provider with your health plan prior to being seen in the office.
It is very important that you bring your Drivers License or other photo ID; Current health care card;and Utility bill or other correspondence showing current residence if the photo ID does not show the patient's current address with you to each office visit. If the patient is a minor, the patient's parent or guardian should bring the information listed above. We ask that you notify us of any changes in address, telephone numbers, marital status or insurance carrier at each visit in order to eliminate delays in the processing of your claims.
