Payment Policies at FPA

Family Practice Associates will be happy to discuss our fees with you in advance for your office visits or procedures to be performed. Please contact the Practice Administrator with any questions you may have.

Important!

All copayments are REQUIRED to be paid at the time services are rendered, without exception. All health plans are not the same and do not cover the same services. In the event your health plan determines a service is to be "not covered" you will be responsible for the complete charge. Payment due at time of service.  All deductibles and out of pocket expenses will be collected at time of service.
